I hereby release, discharge, covenant not to sue, and agree to indemnify and save and hold harmless each of the releasee's from all liability claims, demands, losses, or damages on the minor's account caused or alleged to be caused, in whole or in part by the negligence of the "releasees" or otherwise, including negligent rescue operations. I further agree that if, despite this release, I, the minor, or anyone on the minor's behalf makes a claim against any of the releasees named above, I will indemnify, save, and hold harmless each of the releasees from any litigation expenses, attorney fees, loss liability, damage, or any cost that may occur as the result of any such claim.

Medical Emergencies:

In any event of an accident or emergency I hereby authorize my child to be transported to a hospital for medical treatment and I hold Amazing Grace Dancers and it's representatives harmless in the execution of such. Classroom Rules

NO street shoes on the studio floor
Do not talk while the teacher is talking
Do not leave the studio without permission
Please be respectful of classes in session and wait in the lobby until they are excused
Students are to wait in the lobby for parent pickup
Smile and have fun!

General Policies

Classes run on a month-to-month basis. Tuition is paid on the 1st of every month.

Lobby/Studio Conduct

Please be respectful of the Bella Via facility.
Please keep noise levels to a minimum so as not to distract others.
Please do not use the lobby as a playroom.
Please do not let children play with property of Bella Via.
Please do not dispose of soiled diapers in any of the studio, hallway, or lobby trash bins.
There is to be no running, yelling, or boisterous behavior in the studio lobby, restrooms or hallways at any time.

Class Attendance

Regular attendance is essential for classroom progress to be made.
Parents are encouraged to inform the instructor if a student will be absent from class.
